Mix the cocoa powder, Perfect Keto Unflavored Base, sweetener, and salt in a small bowl until all the ingredients are evenly distributed. Set aside.
Heat the almond or coconut milk in a saucepan over medium heat until it reaches a simmer. Then, add the dry mixture from Step 1 and stir until it is well blended.
Simmer for about 5 minutes, occasionally stirring, until the hot chocolate thickens slightly.
Serve warm, and enjoy!
